About this role

Be the provider an entire community counts on.

For more than two decades, one trusted provider has cared for this East Texas community. Now that provider is heading into a well-earned retirement, and the practice is looking for the right FNP to carry that legacy forward.

This is the kind of family medicine many NPs hope for when they enter the profession. You are the primary care provider for a close-knit town 35 miles from Lufkin. Your patients are not just a chart number. The staff knows them by name, and families come to you from well beyond the immediate area because they trust the care they receive here.

What Makes This Role Different
Real continuity of care: Treat patients from young children to older adults and build relationships that last years
Genuine impact: Be the go-to provider for a community that depends on you
Variety every day: Chronic disease management, acute care, preventive care, routine procedures, and DOT exams
Support patients in recovery: Help patients through the practice’s established suboxone (MAT) program, a meaningful part of family medicine in a small community
A real team behind you: Work alongside collegial physician and advanced practice colleagues

Your Schedule Works for Your Life
Monday through Friday, no evenings or weekends
Phone-only call, with no in-person call requirements
A manageable patient load, because this practice values quality over volume
Time to enjoy life outside the clinic

Compensation and Benefits
$100,000 to $120,000, depending on experience
Paid time off (PTO)
401(k) with employer match
Continuing Medical Education (CME) allowance
Professional liability (malpractice) coverage
Health benefits

A Great Fit If You
Are a board-certified Family Nurse Practitioner who loves full-spectrum primary care
Want to build real relationships with patients and families
Value a collaborative team and a balanced schedule
DOT exam or suboxone experience a plus (not required, can train)

Why This Practice
Deep roots, a strong reputation, and patients who keep coming back. You will have the autonomy and trust of a long-established practice, the support of a strong team, and a schedule that lets you build a life outside of work.

If you are ready for family medicine that is relationship-driven, rewarding, and sustainable, we would love to talk with you.
